For ALL special consideration applications supporting documentation MUST be provided. You can start an application for special consideration before obtaining all of the supporting evidence but is recommended that you have your documentation ready to include with your submission.
All supporting evidence must be submitted within 10 working days of your application being submitted.听 If the supporting documentation is not received within 10 working days of the application being submitted, the application will be withdrawn.听 If additional documentation is then requested it must be submitted as soon as possible, within 10 working days of the application being submitted (where possible).
Supporting documentation should听usually be received prior to the release of results.听 Any such documentation received after the release of results may result in the application being declined.

SC Medical Forms
If your application is based on medical grounds, it must be supported with medical evidence from a registered medical practitioner.听 听You will need to submit a SC Medical Form (2023). You must see your medical practitioner within a reasonable timeframe for them to be able to make a diagnosis, and that they are provided with sufficient information regarding the timing of your assessments/exams (as applicable) to enable the medical practitioner to provide beneficial supporting documentation.听听

Other accepted evidence
If you are submitting an application which is related to a bereavement, appropriate evidence is a death notice from a newspaper or a letter from a medical practitioner, funeral director, minister of religion, religious leader, kaumatua or the police.听 Please note that where the notice or letter does not indicate the relationship with the deceased to the student, you should include as evidence a letter stating the relationship.听 This letter must be supplied by a third party.听
