Supporting Evidence
All Special Consideration Applications聽MUST聽be submitted with supporting evidence.聽 It is important that you submit your supporting evidence at the time you submit your application to avoid unnecessary delays in the processing of your application.
Medical
- Applications made on medical grounds including Covid require a聽fully completed Special Consideration Medical Form, or other detailed medical certificate, as supporting evidence.聽 The SC Medical Form聽MUST聽be completed by a health practitioner, signed by them, dated and have their Health Practice stamp and/or Health Registration number.聽 Please download the聽SC Medical Form (2023) below and have your medical practitioner complete this.
- Electronically signed and unsigned medical forms will聽NOT聽be accepted.
- 驰辞耻听MUST聽see your medical practitioner within a reasonable timeframe for them to be able to make a diagnosis, and the consultation聽MUST聽take place within a period that is聽relevant聽to the assessment item.聽
- The relevant period for a missed examination would be within 48 hours of the examination; and for impaired performance the medical聽forms should be dated within 10 working days (but preferably earlier) of the date of the test or examination.
Bereavement
Supporting evidence in relation to a bereavement can be a copy of the newspaper death notice where your name is listed OR a letter from one of the following:
- Medical Practitioner
- Funeral Director
- Minister of Religion
- Religious Leader
- 碍补耻尘腻迟耻补
- 笔辞濒颈肠别.听
Please note that where a newspaper death notice does not indicate the relationship of the deceased to the student, you will need to include, as supporting evidence, a letter from a third party outlining and confirming the relationship.聽 A third party letter can be provided by a person from the list above.
